Home Based Treatment Team provides an ageless service to people who live in the Bolton Area. We provide an alternative to inpatient admission and support people with severe and acute mental health problems in their homes; whilst undergoing intensive treatment for their mental health needs. An opportunity has arisen for a Band 6 Mental Health Practitioner to join the Home Based Treatment Team in Bolton. One of the staff did say they called on arrival and left a message, but there was no missed calls or messages on my phone. The team is a Mental Health MDT serving the Bolton population, employed by Greater Manchester Mental Health. We provide a rapid assessment and treatment in their own homes or community venues, as an alternative to an inpatient admission. We also provide flexible support tailored to individuals’ needs which can include home visits up to three times a day to assess, listen and talk about difficulties currently being experienced..
You will be expected to carry out in depth assessments in the community and at Royal Bolton Hospital and you will be expected to contribute an MDT formulation and care planning for service users. We engage carers of our service users to help provided and ensure that they have relevant contact information for services and advice on what to do in an emergency or times of concern. We are an age-less service based in The Royal Bolton Hospital and assess any patient who may present with concerns regarding their mental health.
We support patients within the hospital only, this includes A&E and medical wards. The post holder will provide clinical support to other members of staff through the daily MDT meetings and flexibly as the need arises. The post holder may occasionally be called upon to do a physical health examination and perform venepuncture. Home Based Treatment in Trafford
The teams consist of medical staff, nurses, social workers, occupational therapists, pharmacists and support workers. All service users under MHHTT will be allocated a Named Worker who will oversee their care whilst under the team. Service users will be offered a comprehensive assessment of their needs with individualised care plans being developed with them in order to address these needs. MHHTT will engage with other services involved in a service users care in order to develop joint care plans. Our team provides inreach to the inpatient unit and is able to support service users on extended leave from the wards.
